ARTICLE I.-On Ligature of the Carotids. By JAMES MILLER, F.R.C.S.E., Lecturer on Surgery, and one of the Surgeons to the Royal Infirmary of Edinburgh.

Ir has long been understood that deligation of both carotids, simultaneously, is fraught with most serious danger to the brain. And it may be supposed, that, were circumstances to have occurred, seeming to demand such a proceeding, the surgeon either would have altogether refused to operate, or at all events would have done so reluctantly, and only with a faint hope of thereby saving his patient from instant death, otherwise inevitable. The experience of Sir Astley Cooper had shown that carotids and vertebrals might all be safely tied, one after another, but singly, and with an interval between each deligation, supposed to be sufficient for the establishment of free anastomotic circulation; and it seems to have remained a settled point that operative treatment could not, with prudence and safety, be pushed with greater rapidity in regard to these vessels. Such an impression probably received further encouragement by proofs often occurring to the operating surgeon, how simultaneous pressure over both common carotids, with the view of temVOL.
